You had your blood sugar checked.

It was “normal.”

Given the recent surge in interest surrounding "metabolic health," more people are seeking to understand not just their basic labs, but how their daily habits truly influence their internal physiology.

So… does that mean your metabolism is completely healthy?

Not necessarily.

A single blood test gives you one moment in time.

A Continuous Glucose Monitor (CGM) gives you something very different:

A blood test is a photograph. A CGM gives you a movie.

And sometimes, the movie reveals patterns that a single snapshot simply cannot show.

But there is an important distinction:

A CGM does not directly measure insulin resistance, and it cannot diagnose prediabetes or diabetes on its own.

For diagnosis, established tests such as fasting plasma glucose, HbA1c, and the oral glucose tolerance test remain the appropriate tools. Current ADA Standards of Care state that there is not enough evidence to use CGM alone to screen for or diagnose prediabetes or diabetes.(*)

So why use a CGM? (*)

Because glucose patterns can give us valuable information about how your body responds to food, movement, sleep, stress, and meal timing.(*)

And that is where things get interesting.


What Exactly Is a CGM? (And Why It’s More Than Just a Number)

A Continuous Glucose Monitor is a small sensor worn on the body, usually on the upper arm.

Unlike a traditional finger-stick glucose meter, which measures glucose in a blood sample, a CGM measures glucose in the fluid surrounding your cells, called interstitial fluid.

Because it is measuring a different compartment, CGM readings can differ from blood glucose, particularly when glucose is changing rapidly—for example after eating or exercising.

That is why I don't want you obsessing over one number.

The pattern matters more than the individual reading.

Your Metabolism Has a Story to Tell: Decoding Your Unique Responses

Two people can eat exactly the same meal and have very different glucose responses.

This is one of the most fascinating findings from personalized nutrition research.

In a large study of 800 people, researchers continuously monitored glucose and analyzed responses to 46,898 meals. They found substantial differences between individuals in their responses to the same foods.

That means there may not be one perfect breakfast, one perfect carbohydrate, or one perfect meal plan for everyone.

Your response can be influenced by many factors, including:

  • insulin sensitivity

  • meal composition

  • carbohydrate amount and type

  • protein and fat

  • physical activity and muscle activity

  • sleep quality

  • stress and cortisol

  • hormonal changes

  • meal timing

  • what you ate earlier in the day

This is why the question isn't simply:

“Is this food healthy?”

A better question can sometimes be:

“How does MY body respond to this food, in this context?”


How to Spot Patterns, Not Just Numbers (The 3-Step Scan)

A CGM gives you a lot of data.

But more data doesn't automatically mean more insight.

I focus on patterns.

1. What happens after you eat?

Look at the shape of your glucose curve after a meal.

Does it rise gradually?

Does it rise quickly?

How high does it go?

And what happens afterward?

One isolated rise doesn't automatically mean something is wrong.

But if you repeatedly see large excursions after certain meals, that pattern becomes much more interesting.


2. How quickly does glucose recover?

The rise is only part of the story.

Recovery matters too.

Imagine two people who reach a similar glucose peak after a meal.

Person A gradually returns toward their usual level.

Person B remains elevated for much longer.

Those are two very different glucose patterns.

This is one reason I don't focus only on the highest number on the graph.

I want to understand the entire response.


3. How variable is your glucose?

Glucose is not supposed to be perfectly flat.

It naturally moves throughout the day.

But repeated large swings, prolonged elevations, or unexpected lows may provide useful information about your overall glucose pattern.

In clinical CGM interpretation, standardized metrics include mean glucose, glucose variability, time above range, time in range, and time below range. For people with diabetes, international consensus and ADA guidance commonly use 70–180 mg/dL (3.9–10.0 mmol/L) as the standard “time in range” window, alongside other metrics.

Important: these clinical targets were developed primarily for diabetes management. They should not simply be copied and presented as universal “healthy” targets for every person without diabetes.

Decoding the Night: What Your Sleep Data Really Means

Your CGM can also reveal what happens when you're not eating.

And sometimes, nighttime data are incredibly informative.

You might see:

  • a relatively stable overnight pattern

  • a gradual rise toward morning

  • unexplained drops

  • repeated nighttime fluctuations

  • glucose remaining elevated for hours after dinner

One pattern you may hear about is the dawn phenomenon—a rise in glucose around the early morning hours that can occur as part of normal hormonal changes associated with waking.

The important question is not:

Did my glucose rise in the morning?

The better question is:

“Is this pattern consistent, and what does it look like in the context of the rest of my metabolic picture?”


The CGM Reality Check: The Good, the Bad, and the 'Flat-Line' Trap

A CGM can be an incredible learning tool.

But it can also become an incredible source of confusion if you don't know how to interpret it.

THE GOOD

A CGM gives you immediate feedback.

You can see what happens after a meal.

You can compare breakfast with dinner.

You can see what happens when you walk after eating.

You can observe the impact of poor sleep.

You can notice patterns during stressful days.

And instead of guessing, you can start experimenting.

That can be powerful.


THE BAD

More data can sometimes create more anxiety.

Some people start checking their glucose constantly.

A small rise becomes “bad.”

A flat line becomes “good.”

Every meal becomes a test.

And suddenly, eating is no longer about nourishment—it becomes about getting the “perfect” graph.

That is not the goal.

If CGM data are making you anxious, obsessive, or overly restrictive around food, the technology may be doing more harm than good.

Don't optimize the graph. Optimize the person.


THE UGLY: The Flat-Line Trap

A flat glucose graph can look beautiful.

But beautiful doesn't always mean healthy.

For example, alcohol can suppress the liver's normal production and release of glucose. That can sometimes make the glucose curve look deceptively flat.

That does not mean alcohol improved your metabolic health.

Similarly, a meal very high in fat may produce a delayed glucose response because digestion and gastric emptying can be altered.

So don't judge a meal simply by what happens during the first hour.

Look at the whole picture.


When to Trust (and When to Question) Your CGM

CGMs are incredibly useful—but they are not infallible.

Here are a few situations where you should pause before interpreting the graph.

Compression lows

If you sleep directly on your sensor, pressure can sometimes produce an artificially low reading.

Rapid glucose changes

CGM glucose and blood glucose can differ during periods of rapid change, such as after meals or exercise.

Vitamin C and other interfering substances

Some CGM systems can be affected by substances such as vitamin C, and the effect can depend on the device and dose. Always check the manufacturer's guidance for your specific sensor.

A double spike

You may sometimes see glucose rise, begin to fall, and then rise again.

A double peak can occur for several reasons, including meal composition, digestion, and the timing of glucose absorption.

By itself, a double peak is not a diagnosis of insulin resistance—and it is not automatically proof of excellent metabolic health.

The context matters.


Is That ‘Healthy’ Food Really Working for You?

Exactly.

A food can be nutritious and still produce a larger glucose response in a particular person.

Take oatmeal, for example.

Oatmeal may be considered a healthy breakfast, but some people may experience a much larger glucose response to it than they expected.

That doesn't mean oatmeal is “bad.”

It means your individual response is worth understanding.

This is the foundation of personalized nutrition research: people can respond differently to the same foods.

Turn Your Data Into Action: The 5-Day Metabolic Experiment

This is where I believe CGM becomes much more useful.

Instead of simply watching numbers, you can use your sensor to run small, structured experiments.

Experiment 1: Establish your baseline

Observe your usual patterns before changing everything.

Experiment 2: Test movement

Compare a meal followed by sitting with the same type of meal followed by a short walk.

What changes?

Experiment 3: Test meal composition

Compare different combinations of protein, carbohydrates, fiber, and fat.

How does your glucose respond?

Experiment 4: Test sleep

Compare a good night's sleep with a poor night's sleep.

Does your glucose pattern change the following day?

Experiment 5: Test meal timing

Look at what happens when you change your eating window or avoid late-night eating.

The point isn't to create a perfect graph.

The point is to learn how your body responds.

This distinction matters.

A CGM can show you patterns.

It cannot tell you your fasting insulin.

It cannot directly measure insulin resistance.

It cannot diagnose diabetes or prediabetes.

And it cannot replace appropriate laboratory testing or medical care. Current ADA guidance continues to recommend established blood-based tests for screening and diagnosis.

But it can help you ask better questions.

And sometimes, asking the right question is where real change begins.
